If the event is being held at the temple, submit a Room Request/Setup Form as well.
If at Temple Judea, specify room(s)
Include name and contact information for each person.
Examples: families with elementary school-age children, adults only, all ages. Specify if open to non-members.
Please provide as much information as possible. Your description will be used to create event publicity. Short phrases or bulleted lists are fine — the communications team will do the rest!

Examples: photo of guest speaker, or photo from same event in previous year. You can also email image files or photos to or drop off pictures at the temple office.
